Primary Application
The first step is to submit your primary application to the Physical Therapy College Application Service (PTCAS) by the deadline. Submitting materials early will ensure timely processing and help avoid delays.
Visit the PTCAS website today to confirm primary application procedures and deadlines and to take the first step toward becoming a College of Allied Health Professions - Doctor of Physical Therapy student!
Secondary Application
The second step in the application process is to submit your secondary application directly to WesternU by the postmark deadline. To assist in compiling your secondary application, we've created a checklist that explains what you need to do to complete your secondary application. Please review the checklist carefully to ensure that you submit all required documents in the appropriate manner along with your secondary application.
Application Documents
You may complete your application by saving the blank PDF documents below, typing in your information and then printing your final drafts, or you may request hard copy application forms. You may also wish to print blank copies of all forms and complete your first draft by hand. If needed, you can download the latest version of Adobe Acrobat and/or view download tips.
Please note that the information entered into these forms cannot be saved, so do not close the forms until you are completely satisfied with the printed results!

Admissions Process
To give you peace of mind, we will e-mail you within two weeks of when each of your applications has been received. On-campus interviews of selected applicants are typically held in Pomona, CA in the spring. Should you be invited to interview, you can expect to hear from us in January.
